Painting prep advice

I am about to paint the internal walls of a ~90 year old property. On some sections of the wall, there is old layers of paint that has peeled and not been prepared correctly and instead painted straight over. What would be the best course of action to achieve a flat surface? I was thinking :
1 - scrap all paint + prime
2 - try to fill the gaps + prime
3 - or would priming alone be enough?
